Submit a Claim - Instructions

You can submit a claim for reimbursement for unreimbursed out-of-pocket costs incurred on or before May 31, 2024, to replace and install a charcoal filter in a Subject Vehicle; and/or flush the evaporator in a Subject Vehicle. You can also file a claim for up to $100 for unreimbursed out-of-pocket expenses to replace and install a charcoal filter in a Subject Vehicle incurred after May 31, 2024.

The deadline to submit a claim for reimbursement of out-of-pocket expenses incurred on or before May 31, 2024, is May 31, 2025. For out-of-pocket expenses incurred after May 31, 2024, the deadline is May 31, 2025 (for Model Year 2012-2013 Subject Vehicles) or May 31, 2026 (for Model Year 2014-2015 Subject Vehicles).

Documentation:

Documentation must be provided to support your claim(s). Documents should be clear, readable copies, as anything you submit will not be returned to you. Acceptable documentation includes receipts, voided checks, bank statements, or other documents showing the amount of your losses and/or a detailed narrative description.

The best way to show you incurred eligible out-of-pocket expenses is to enclose invoices, service records, repair orders, or any other documents that show:

  • Proof of ownership, which includes VIN, make, and model
  • Repair date
  • Type of eligible repair performed (i.e., replacement of charcoal filter or evaporator flushing)
  • Proof of payment and total amount paid (for both parts and labor)
  • Facility name, address, and phone number that performed the repair

If you are filing online, please have this information ready before you start to file, as your claim will not be saved if you have to come back and finish at a later time. Documents that are uploaded must be less than 20 MB per file and in one of these formats: jpg, jpeg, png, gif, tif, tiff, doc, docx, xls, xlsx, pdf, txt, rtf, or zip. If you are unable to electronically upload a copy of your documents as part of the online Claim Form, you will need to mail a printed Claim Form along with your documentation to the Settlement Administrator.


How to File Online:

As part of filling out your Claim Form you will be asked to provide information about the repairs, including the repair date, amount of expense, and type of eligible repair (i.e., replacement of charcoal filter or evaporator flushing). You must also provide proof of payment and proof of ownership or lease.

After submitting your completed claim online, you will receive an email with a Confirmation Code for your completed submission. Be sure to keep your confirmation email and Confirmation Code and refer back to them if you have any questions about your Claim Form.
